At least four doses of diphtheria, pertussis, tetanus and poliomyelitis vaccine, with the last dose administered on or after the fourth birthday.
Measles, mumps and rubella: To enter the sixth grade, a second dose of vaccine administered after the first birthday is required. Children entering sixth grade must have the varicella vaccine or proof the child has had chickenpox.
South Carolina
1. Three doses of any combination of DTP, DT, DTP-Hib, DTaP or Td vaccine, with at least one dose on or after the fourth birthday.
2. Three doses of any combination of oral or inactivated polio vaccine, with at least one dose on or after the fourth birthday.
3. Two doses of rubeola (measles) vaccine, with both doses received on or after the first birthday and separated by at least one month.
4. One dose of rubella (German measles) vaccine received on or after the first birthday.
5. One dose of mumps vaccine received on or after the first birthday.
6. Three doses of hepatitis B vaccine.
7. One dose of varicella (chickenpox) vaccine on or after the first birthday or positive history of the disease for all children admitted to kindergarten and first, second and third grades.
